Patient Coordinator - National Hereditary Haemorrhagic Telangiectasia (HHT) Centre
Part Time Hours (17.5hrs per week), Temporary Contract
Principal Duties and Responsibilities include:
- National Point of Contact - sources of referrals: Serve as the central contact for patients nationwide enquiring about the condition. Collect and manage patient details, medical histories, and referral information.
- Appointment Coordination: Arrange and schedule genetic testing, imaging studies, and related diagnostic procedures.
- Meet and liaise patients in person during their hospital visits, to provide support and coordination.
- Multidisciplinary Communication: Act as the key liaison between patients, consultants, other clinical staff, and other hospital departments to ensure smooth patient journeys.
- Record Keeping & Data Management: Maintain and update patient records to ensure accurate follow-up for future testing and monitoring.
- Provide ongoing communication and information to patients regarding annual reviews and follow-up care.
- Provide service development support by participating in service development, process improvement, and patient pathway optimization as required.
- Liaise with Patient Support Groups, International HHT organizations and HHT Centres elsewhere.
- Other duties as appropriate to the post which may be assigned from time to time.
Candidate Criteria - Desirable:
- Background in healthcare, nursing, clinical genetics, or patient services would be hugely beneficial.
- Experience working in patient-facing roles, including coordination or administration in a hospital setting, would be an advantage.
- Strong organizational and time management skills, with capability to handle multiple patient pathways simultaneously, would be necessary.
- Ability to work independently with minimal supervision while also collaborating effectively with clinical teams, would be necessary.
- Understanding of the complex nature and multi-system features of HHT would be necessary for effective conduct of the coordinator's role (this can be learnt by an appropriately trained candidate during induction).
