Full job description

Hollycare Health and Disability Services is extending its HSE-funded home care and disability support service into Kerry. The Kerry care coordinator will hold day-to-day operational and accountability for the service area, including local recruitment, rostering, care plan oversight, and staff supervision. The role reports directly to the Operations Manager and forms part of Hollycare's wider management structure.

Key Responsibilities

Service Delivery & Client CareService Delivery & Client Care

  • Oversee the day-to-day delivery of home care and support services to clients, ensuring care is delivered safely, consistently, and in line with each client's care plan.
  • Conduct client reviews and initial assessments in conjunction with the Operations Manager, escalating changes in client need promptly.
  • Act as the first point of contact for clients and families regarding day-to-day service queries or concerns.

Staffing, Rostering & SupervisionStaffing, Rostering & Supervision

  • Coordinate and manage the local staff roster, ensuring adequate cover for all allocated care packages.
  • Provide direct line supervision to Care Support Workers, including regular check-ins and supervisions.
  • Support recruitment and onboarding recruitment Lead.

Compliance, Quality & ReportingCompliance, Quality & Reporting

  • Maintain accurate records of client care, staff rosters, and incidents, and submit regular reports to the Operations Manager.
  • Report any safeguarding concerns immediately and directly in line with Hollycare's Safeguarding Policy, independent of the standard HR line.
  • Support internal audits and HSE monitoring visits relevant to the service.

Communication & EscalationCommunication & Escalation

  • Provide a weekly summary report to the Operations Manager covering staffing levels, client status, incidents, and any risks.
  • Escalate operational, staffing, or safeguarding issues promptly through the appropriate line.
  • Represent Hollycare professionally in dealings with clients, families, and any local HSE or community contacts.

Person Specification

EssentialEssential

  • Minimum 2 years' experience in home care, disability support, or a related health and social care setting.
  • Experience supervising or coordinating a team, even informally (e.g. senior carer, shift lead).
  • Full clean driving licence and access to own transport, with willingness to travel across the service area.
  • Satisfactory Garda vetting.
  • Strong organisational skills with the ability to manage rostering and competing priorities.
  • Clear, confident communicator, comfortable liaising with clients, families, and staff.
  • Working knowledge of safeguarding principles for older adults and adults with disabilities.

DesirableDesirable

  • QQI Level 5 in Healthcare Support or equivalent.
  • Previous experience working within a HSE-funded service.
  • Experience with care management or rostering software.
  • Local knowledge of the area and community services.
